The plain fact
Most people who get a denial letter never appeal. The letter tells you that you can — and then leaves you staring at a blank page with a deadline running. Most people give up right there. That is what the system counts on.
Public federal data says the ones who do appeal fare differently: in 2024, about 8 in 10 Medicare Advantage denials that were appealed ended with the denial overturned — while only about 1 in 9 denied requests were appealed at all (KFF’s analysis of CMS data, published January 2026). That is a fact about the population of appeals, not a forecast for any one case — including yours. Nobody can promise what an appeal will do. The appeal has to exist to have a chance.
What you get
Email us your denial letter and bill; within one business day you get back a complete packet of documents, ready for you to fill, sign and file:
- The appeal letter — written in your voice, citing the rules and plan language your own plan has to follow, addressed to the right appeals office, with a section asking the plan for the exact criteria and reviewer credentials it used to deny.
- The external-review request — the next step if the plan says no again, with every field the form asks for gathered for you in advance.
- The attachment checklist — what goes in the envelope, in what order.
- The filing instructions — where it goes, how to send it so you have proof of delivery, and your real deadline, worked out from the date on your letter.
You fill the brackets, check every date and number against your own letter, sign it, and file it. It stays your appeal the whole way — we just build the paperwork so it is complete the first time.
What this is not — read this part
- This is not legal advice. Claim Pack is a document-preparation service — the same category of help as letter templates or tax-form software. We are not lawyers and not a law firm.
- We never act for you. We never contact your insurer, never negotiate, never represent you, and never file anything on your behalf. You file everything yourself.
- No outcome promises. Nobody can promise an appeal wins, and we do not. Many appeals fail. The public statistics above describe appeals in general, never your case.
- No percentage, ever. You pay one flat fee for the documents. We never take a share of anything that comes back.
What it costs
Flat price, paid once, for the documents:
- $49 — standard packet: one denial, one claim.
- $99 — multi-claim denials or second-level appeals.
- $149 — Medicare multi-level cases or multi-party matters needing custom-built sections.
Same-day turnaround when your deadline is under 72 hours. And one rule we hold to: if we cannot build a usable packet for your denial, we tell you plainly, refund in full, and point you at the better free resource — your state insurance department, SHIP for Medicare questions, or legal aid. Some problems need a lawyer, and when that is the better road we say so.
How it works
- You email your denial letter (a phone photo is fine) and the bill or EOB.
- We ask a few short questions by reply — plan type, state, dates, what happened.
- We confirm your denial fits what we can build, and send a payment link for the flat price.
- Within one business day of payment (same day if your deadline is inside 72 hours), your packet arrives by email, ready to fill and file.
To start: email claims@orbitalfour.com with “denial” in the subject line and your denial letter attached. Nothing is charged until we have confirmed the packet is buildable.
Your documents
Denial letters and bills are personal, health-adjacent information. Yours are deleted from our systems 30 days after your packet is delivered. They are never shared with anyone and never used for marketing. That is the whole policy.
Questions
Is this legal advice? No. It is document preparation — the same category of help as letter templates or tax software. Every packet says so, in plain words, on its first page.
Will my appeal win? Nobody knows, and anyone who promises is selling you a story. What we can say: a complete, on-time, well-documented appeal is the best position available, and most denials never face one.
My deadline is in three days. Say so in your email — same-day turnaround is built for exactly that.
What if my denial is strange? If it does not fit our templates, we say so and do not charge. We will also tell you when the better road is your state insurance department or a lawyer.
Who is behind this? Claim Pack is an insurance-paperwork product of Orbital Four, a small document-services company in Toronto, Canada.
Start with one email
Send the denial letter to claims@orbitalfour.com — “denial” in the subject, letter attached. You will hear back the same business day with either the first questions or an honest “this one needs a lawyer instead.”
